- Established Patient Visit – $69
- Follow-Up Visit (Same Condition) – $59
- Quick Consult (Medication Question – up to 5 minutes) – $35
Chronic Care Telehealth
- Hypertension Follow-Up (I10) – $59
- Diabetes Follow-Up (E11.9) – $69
- Hyperlipidemia Follow-Up (E78.5) – $59
Administrative Visits
- Medication Refill Visit – $39
- Work or School Excuse Note – $35
